The following information is needed at the time of the request:
> Agency name and callback number
> Nature of the incident (accident, fall, medical)
> Location of the landing zone if available
> Radio frequency and PL(channel guard) of the vehicle/personnel securing the landing zone

Within 5-10 minutes, the following additional information should be relayed
> Age and sex of the patient
> Level of consciousness, overview of injuries/illness
> Landing zone location (Predesignated number, street address, or intersection)
> Unit (vehicle or personnel radio designation) securing the landing zone
